This can be a little hairy your first time out, but you needn't worry.  See 
below for the appropriate responses...

Most definately.  These entry forms don't need to be filled out completely.  
The most important parts would be ingredients.  In the past I've partially 
filled out forms, leaving out gravities, CO2 volumes, yeast varieties, etc.  
In short, the more info the better, but if you don't have it, you don't have 
it.  Fill out what you can and let your beer speak for itself.
